Technological innovation in laser photobiomodulation and vacuotherapy for low back pain: a pilot study

Abstract

Approximately 80% of the global population experiences or will experience back pain. This study aimed to investigate the acute and chronic effects of the combined application of laser photobiomodulation (PBM) and vacuotherapy in individuals with low back pain. Participants were divided into a Treated Group (laser PBM + vacuum therapy) and a Placebo Group (laser off). To evaluate acute (1st session) and chronic (7 sessions) effects, pre- and post-treatment assessments were performed. These included algometry for pain threshold, dynamometry for isometric trunk strength, goniometry for lumbar range of motion (flexion/extension), and the finger-to-floor flexibility test. There was no placebo effect observed. Acute and chronic effects were observed only in the Treated Group. The acute effects were increased pain thresholds and lumbar flexibility. The chronic effects were higher pain thresholds, greater isometric trunk strength during flexion and extension, and greater lumbar flexibility. No significant differences were observed in angular measurements of lumbar spine flexion and extension. In the current study, the therapeutic effects may be related to myofascial release, increased blood flow, analgesia, and anti-inflammatory action. Therefore, the first session of laser PBM applied simultaneously with vacuum therapy showed pain relief and greater mobility of the lumbar spine. Moreover, after completing seven sessions of combined therapy, there was a reduction in pain and improvements in strength and flexibility.
